Summary Care Record – Patient Access Online

With Patient Access, you can now access a summary of your medical records at home, work or on the move — wherever you can connect to the internet. What’s more, because Patient Access is a 24 hour online service you can do this in your own time, day or night. Ask at reception for more information or to sign up.

Access to your medical records

You are able to request access to your medical records. This can be done either by attending the practice and viewing your computerised records on screen (the majority of your records will be computerised) and your old paper records, you will need to be accompanied by a member of staff when accessing your records.

Copies of your records

In addition, you can request copies of your records. We can supply these on an encrypted CD, if you wish to have electronic copies, or on paper, provided that you collect the paper copies in person. We are not able to post paper copies to you.
