Named Accountable GP for all Patients
From the 1st of April 2015, the practice is required under the terms of the latest GP contract to allocate all patients (including children) a named accountable GP who is responsible for patients overall care at the Practice and who will take lead responsibility for the coordination of all services required under the contract.
Having a named GP does not prevent you seeing any other doctor in the practice for your choosing. Your named GP will not be available at all times and if your needs are urgent, you may need to discuss them with an alternative doctor.
If you would like to know who your named GP is, please ask during your next consultation or at reception.

If you wish to change your named accountable GP, please inform us and we will make every effort to accommodate your request.
In addition, for patients aged 75 and over the named GP will also:
- work with relevant associated health and social care professionals to deliver a multi-disciplinary care package that meets the needs of the patient
- ensure that these patients have access to a health check
